Job summary

UnitedHealthcare is seeking a Senior Clinical Transformation Consultant to lead multi-disciplinary practice transformation within Accountable Care Organizations and other population health programs. You will guide practices toward quality, efficiency and savings targets, acting as the clinical point of contact for progress updates.

This role reports to the Regional Director and mentors other transformation consultants. If located within commuting distance of St.

Qualifications

• Bachelor's degree or 9+ years in clinical quality improvement or population health.
• 5+ years of clinical quality improvement or transformation in population health programs.
• 2+ years of experience interpreting and using clinical data analytics and outcomes measurement.
• Experience working with and presenting to senior leadership.
• Strong presentation skills and relationship-building with internal and practice teams.
• Proficient with Word, Excel, Outlook and PowerPoint.
• Access to high speed internet for home office setup.
• Willingness to travel 10-15% for face-to-face meetings.
• Reliable transportation to travel to client/patient sites.

Responsibilities

• Deploy UHC's Clinical Support program at the practice level, including training on reporting tools and physician portal usage.
• Build and maintain relationships with practice leadership and clinical influencers.
• Facilitate practice improvement meetings to monitor milestones and progress on action plans.
• Develop performance-improvement strategies based on performance analyses and metrics.
• Design and implement practice transformation action plans to meet milestones.
• Monitor milestone progression and ensure accountability for completion.
• Analyze cost, utilization and quality data to assess practice performance.
• Use data trends to drive proactive improvement strategies (PDSA cycles).
• Assist practices in creating workflows to optimize care delivery and adopt best practices.
