Job summary

Alliance Health is seeking a Practice Transformation Specialist to support AMHs and CMAs across the NC region in improving patient outcomes through CQI, data-informed decision making, and provider coaching. The role is remote with occasional in-person meetings at Alliance offices as needed.

The candidate will educate clinicians and CINs on 1915(i) processes, leverage data dashboards, and guide care management toward value-based models, while collaborating with IT and interdisciplinary teams.

Qualifications

• Bachelor’s degree in nursing, social work, or other human services related field required.
• Five (5) years of post‑degree healthcare or managed care experience preferred.
• Experience leading healthcare quality and process improvement initiatives is preferred.

Responsibilities

• Provide support to network providers to evolve workflows and care management processes.
• Educate practice staff and CINs on CQI principles and PDSA cycles with data-driven insights.
• Meet with providers to review performance reports and present data in an understandable format.
• Collaborate with AMHs and CMAs to integrate data across systems for better outcomes.
• Assist in data reporting and gaps analysis to strengthen network care management.
