Job summary

UnitedHealthcare is seeking a Senior Clinical Transformation Manager to lead multi-disciplinary practice transformation within physician collaborations, driving quality, efficiency and cost improvements across Accountable Care and Population Health programs. This remote role may require 10–15% travel and reports to the Regional Director.

Qualifications

• Bachelor's degree or 9+ years of experience in clinical quality, analytics, or transformation with population health programs.
• 5+ years of combined experience in related transformation initiatives.
• 2+ years of experience interpreting and using clinical data analytics to drive change.

Responsibilities

• Lead deployment of clinical support programs at practice level and educate on reporting tools.
• Build relationships with practice leadership and clinical influencers.
• Facilitate practice improvement meetings to monitor progress on transformation plans.
• Develop strategies based on performance analyses to monitor outcomes.
• Design and implement action plans for contractually required milestones.
• Monitor milestone progress and ensure accountability of practices.
• Analyze data to assess practice performance and identify trends.
• Collaborate with teams to integrate care management programs.
