Job summary

UnitedHealthcare is seeking a Clinical Transformation Consultant to drive practice-level transformation within Accountable Care and population health programs. You will guide multi-disciplinary teams, analyze clinical data, and collaborate with senior leaders to achieve quality, efficiency, and cost goals.

The role supports practices in achieving milestones, deploying reporting tools, and integrating technology into workflows. Remote options available for IL/WI locations; travel ~15% is expected.

Qualifications

• 4+ combined experience with improving clinical quality, health care analytics, or driving clinical transformation initiatives.
• Experience interpreting and utilizing clinical data analytics to drive change.
• Experience working with senior leadership and presenting to stakeholders.
• Strong Microsoft Office skills (Word, Excel, Outlook, PowerPoint).
• Ability to build relationships with internal and practice teams.

Responsibilities

• Lead deployment of Clinical Support program at the practice level with reporting tools and portals.
• Build relationships with practice leadership and clinical influencers.
• Facilitate practice improvement meetings to monitor progression against milestones.
• Develop data-driven improvement strategies with measurable outcomes.
• Design and implement performance improvement initiatives to meet milestones.
• Monitor milestone progress and ensure accountability.
• Analyze cost, utilization, and quality data to assess practice performance.
• Collaborate with teams to address issues using rapid cycle improvement (PDSA).
• Educate and deploy technology tools to support transformation.
