Job summary

Aledade PBC seeks a Practice Transformation Specialist to support independent primary care practices in implementing value-based care initiatives and improving patient outcomes.

The role focuses on direct practice support, training staff on care processes, and building relationships with physicians and healthcare leaders to enhance quality and financial performance. Remote-first with some travel.

Qualifications

• Graduate of an accredited college or university.
• 4-6 years of significant and relevant work experience in medical practice management.
• 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes.

Responsibilities

• Direct Practice Support: Implement products and execute ACO initiatives with direction from Market Leadership.
• ACO Training & Development: Train office staff and providers on processes and technologies; assess training needs.
• Customer Success: Listen to practices concerns and relay information to management to identify improvement opportunities.
