Job summary

UnitedHealth Group is seeking a Senior Clinical Transformation Consultant to lead the clinical management of physician practices within UnitedHealthcare’s ACOs, value-based programs, PCMH, or population health initiatives. Qualifications

• Bachelor's degree or 9+ years of experience in clinical quality improvement or population health programs.
• 5+ years of experience with improving clinical quality or driving clinical transformation initiatives.
• 2+ years of experience interpreting and utilizing clinical data analytics.
• Experience working with and collaborating with senior level leadership.
• Demonstrated experience leading groups and solid presentation skills.
• Solid Microsoft Office skills (Word, Excel, Outlook, PowerPoint).
• Access to high speed internet for home office setup.
• Willingness to travel 10-15% for face-to-face meetings.
• Access to reliable transportation to travel to client/site locations.

Responsibilities

• Deploy UHC Clinical Support program at the practice level, including reporting tools and physician portal delivery.
• Build and maintain relationships with practice leadership and clinical influencers.
• Facilitate practice transformation meetings to monitor progress and milestones.
• Develop strategies based on performance analysis to drive improvement and set outcomes.
• Design and implement transformation action plans to achieve contract milestones.
• Monitor milestone progress and ensure accountability for outcomes.
• Analyze cost, utilization, and quality data to assess practice performance.
• Collaborate with stakeholders to address issues using rapid cycle improvements (PDSA).
• Assist in creating workflows to optimize care delivery and evaluate outcomes.
• Educate and deploy technology tools to support Practice Transformation and integrate into workflows.
• Build relationships with UHC Clinical team, Medical Directors, and other analysts.
