Job summary

CVS Healthis seeking a Population Health Care Manager RN to coordinate care for chronic conditions and psychosocial needs. This telework role, based in Maricopa County, AZ, requires strong clinical and case management skills and collaboration across providers to improve member outcomes.

The role emphasizes education of members and providers, data analysis, and development of personalized care plans with travel up to 25% to provider locations.

Qualifications

• Must reside in Maricopa County, AZ
• Active and unrestricted AZ state RN licensure
• 5+ years' experience as an RN
• 1+ year of experience speaking with members in-person, over the phone, or virtually and delivering presentations
• Willingness to travel within Maricopa County 10%-25% of the time

Responsibilities

• Develop and implement pilot programs for population health initiatives.
• Analyze member data to understand health status.
• Present data to health plan leadership as needed.
• Create educational resources and presentations on chronic conditions and management initiatives.
• Assess member needs and coordinate services across providers.
• Advocate for members to ensure access to resources and care.
• Use secure messages, video calls, and other platforms for care coordination.
• Assist providers with outreach and follow-ups for chronic conditions.
• Participate in community-based care management initiatives.
• Educate members and providers on self-management and motivational interviewing.
• Develop resources to optimize healthcare using community resources.
• Identify process and documentation improvements and develop action plans.
• Perform care management with personalized care plans and documentation.
