Job summary

CVS Health is seeking a Case Management Coordinator to work remotely with travel to visit members in Maricopa County, Arizona. You will coordinate, document, and communicate all aspects of the utilization/benefit management program, applying evidence-based care and medical necessity criteria to support members.

The role requires 2+ years in case management with SMI or elderly populations, residency in Maricopa County, and willingness to travel up to 50%.

Qualifications

• 2+ years of case management experience with SMI and elderly populations.
• Willingness to travel up to 50% of the time within Maricopa County, AZ.
• Bachelor’s degree or RN license in AZ.

Responsibilities

• Coordinate and document care management plans and member evaluations.
• Assess eligibility and align benefits with programs/services.
• Engage members through coaching and motivational interviewing to support health goals.
• Escalate quality issues and collaborate with providers.
