Job summary

CVS Health is seeking a Population Health Care Manager RN in Arizona to coordinate care for chronic conditions, educate providers, and advocate for members across healthcare teams.

The role is telework-based from Maricopa County with 10–25% travel to provider locations. Full-time hours are 40 per week, Monday–Friday, 8am–5pm AZ time, with orientation and training leading to flexibility.

Qualifications

• Resides in Maricopa County, Arizona.
• Active and unrestricted AZ state RN licensure.
• 5+ years' experience as an RN.
• 1+ year of experience presenting to diverse groups and delivering presentations.

Responsibilities

• Develop and implement pilot programs for population health initiatives.
• Analyze member data to understand health status and present findings to leadership.
• Create educational resources on chronic conditions and management initiatives.
• Assess member needs, coordinate services across providers, and develop personalized plans.
• Advocate for members to ensure access to resources and appropriate care.
• Utilize secure messages, video calls, and files for care coordination.
• Assist providers with outreach and follow-ups for members with chronic conditions.
• Participate in community-based care management initiatives.
• Educate members and providers on self-management and treatment adherence.
• Develop resources to optimize healthcare using community resources.
• Identify areas for process improvement and develop action plans.
• May perform care management with personalized plans and documentation.
