Qualifications

• 2+ years of case management experience with individuals designated as having a serious mental illness (SMI) and elderly.
• Must reside in Maricopa County, Arizona.
• Willingness to travel up to 50% of the time within Maricopa County, Arizona.

Responsibilities

• Evaluate members and assess needs/eligibility to plan case resolution.
• Coordinate and monitor care plans and progress.
• Identify, escalate and address quality of care issues and barriers.
