Job summary

CenterWell Senior Primary Care, part of the Humana family, seeks a Physician Collaborator for a fully remote role supporting patients with recent fractures. You will collaborate with APPs to close care gaps and coordinate monthly chart reviews.

Reporting to the Associate Director, you will guide APPs to ensure high-quality patient care from home. This role includes travel to Humana offices occasionally for training.

Qualifications

• Unrestricted medical license required.
• Graduate of MD/DO program from an accredited university.
• Board certification or board eligibility.
• 2+ years of experience collaborating with mid-levels.
• Work-from-home internet requirements and dedicated space to protect PHI/HIPAA information.

Responsibilities

• Collaborate with APPs to close gaps in care and ensure access to needed services.
• Complete regular chart reviews and coordinate monthly meetings.
• Serve as a clinical resource for APPs to ensure quality patient care.
