Job summary

CenterWell Senior Primary Care seeks a Physician Collaborator to support APPs in closing care gaps for patients who recently fractured; this is a fully remote role reporting to the Associate Director. Occasional travel to Humana offices may be required for training or meetings.

The role requires an MD/DO, unrestricted SC license, board certification or eligibility, and at least two years collaborating with mid-levels in a care team.

Qualifications

• Must have unrestricted medical license and reside in South Carolina.
• Graduate of MD/DO program from an accredited university.
• Board certification or Board Eligible.
• 2+ years of experience collaborating with mid-levels (APPs).

Responsibilities

• Collaborate with APPs to close gaps in care and coordinate wraparound services.
• Complete regular chart reviews and coordinate monthly meetings.
• Serve as a resource for APPs to ensure quality patient care.
