Job summary

Humana Inc. through CenterWell Senior Primary Care is seeking a Virtual Physician Collaborator to provide remote wraparound care for patients who cannot see their PCP, reporting to the Associate Director.

The role is fully remote and requires approximately 5 hours per month. The position emphasizes collaboration with Nurse Practitioners and clinical leaders to improve care quality and ensure timely access to necessary medications.

Qualifications

• Current and unrestricted medical license in LA, NC, MS, KY, MO, or IN.
• If supervising in LA, MS, or IN you must reside in that State.
• Graduate of an accredited MD or DO program.
• Board certification or Board Eligible.
• 2+ years telemedicine/telehealth experience.
• 2+ years of experience as a Collaborating Physician.

Responsibilities

• Perform regular chart reviews and coordinate monthly meetings.
• Serve as a resource for the advanced practice provider to ensure quality patient care.
• Collaborate with clinical leaders on quality of care, outcomes review, and records issues.
