• Minimum 2+ years collaborating with mid-level providers.

Responsibilities

• Collaborate with APPs to close gaps in care and ensure access to needed services.
• Complete regular chart reviews and coordinate monthly meetings.
• Serve as a resource for APPs to ensure quality patient care.
