• 2+ years of experience collaborating with mid-levels.
• Work from Home.

Responsibilities

• Collaborate with the APP to close gaps in care and ensure access to needed services.
• Complete regular chart reviews and coordinate monthly meetings.
• Be a resource for APPs to ensure quality patient care.
• Use your skills to make an impact.
