Job summary

Humana Inc. is seeking a Physician Collaborator for CenterWell Senior Primary Care. This fully remote role reports to the Associate Director and supports APPs delivering wraparound services for fracture patients who cannot see their PCP.

You will collaborate with APPs, perform chart reviews, coordinate monthly meetings, and serve as a resource to ensure high-quality patient care, with travel to Humana offices for training as needed.

Qualifications

• MD/DO degree required.
• Unrestricted medical license in South Carolina.
• Board certified or board eligible.
• 2+ years of experience collaborating with mid-level practitioners.

Responsibilities

• Collaborate with APPs to close gaps in care and ensure access to needed services.
• Complete regular chart reviews and coordinate monthly meetings.
• Be a resource for APPs to ensure quality patient care.
• Utilize clinical skills to impact patient outcomes.
