Job summary

Humana Inc. is seeking a Physician Collaborator for CenterWell Senior Primary Care. This remote role reports to the Associate Director and supports patients via virtual wraparound services alongside APPs, focusing on fracture care and continuity of care.

Applicants must hold an unrestricted MD/DO license in the specified states and have board certification or eligibility, plus at least 2 years collaborating with mid-level clinicians. Occasional travel to Humana offices may be required.

Qualifications

• Must have unrestricted medical license in KY, TN, MO, NC, TX, IN.
• Graduate of MD/DO program from an accredited university.
• Board certification or Board Eligible.
• 2+ years of experience collaborating with mid-levels.

Responsibilities

• Collaborate with APPs who provide virtual wraparound services to support patients who have had fractures in the last 6 months.
• Coordinate care for patients who cannot see their PCP in person.
• Report to Associate Director and maintain compliance with clinical governance.
