Job summary

Upperline Health, Inc. is seeking a Remote Pharmacy Technician to support our clinical pharmacy program in a fully remote, Monday–Friday schedule. You will conduct telephonic patient outreach, gather medication histories, and assist with medication adherence initiatives under the supervision of a licensed Pharmacist.

Qualifications

• Active Florida pharmacy technician registration in good standing.
• Minimum of three (3) years of pharmacy technician experience with medication reconciliation.
• Strong communication, attention to detail, and patient-focused service.

Responsibilities

• Obtain, collect, and verify complete medication histories via outbound calls.
• Perform medication reconciliation and escalate questions to the supervising Pharmacist.
• Promote medication adherence through patient outreach and 90-day prescription coordination.
• Document outcomes in the EHR and care management platform.
• Research affordability options for pharmacist review.
• Coordinate with care team, providers, and pharmacies to resolve access barriers.
• Maintain HIPAA-compliant remote documentation.
• Perform other duties as assigned by the Pharmacist.
