Job summary

Upperline Health Plus is seeking a full-time Remote Pharmacy Technician to support our clinical pharmacy program. You will conduct telephonic patient outreach, gather medication histories, and support medication adherence initiatives in a fully remote role.

Under the supervision of a licensed Pharmacist, you will focus on medication reconciliation, outreach, and quality gap closure while ensuring privacy and HIPAA compliance in a remote setting.

Qualifications

• Minimum of three (3) years of pharmacy technician experience.
• Strong understanding of pharmacy terminology, prescription medications, OTC products, and dietary supplements.

Responsibilities

• Conduct outbound telephone outreach to patients to obtain and verify medication histories.
• Conduct medication reconciliation and escalate discrepancies to the supervising Pharmacist.
• Promote medication adherence through patient outreach and refill reminders.
• Assist with quality measures gap closure and document outcomes in the EHR.
• Research medication copay assistance programs for pharmacist review.
• Collaborate with care team and pharmacies to coordinate medication-related needs.
• Maintain documentation in the EHR and generate routine reports.
• Ensure HIPAA compliance in a remote work environment.
• Perform other duties as assigned by the supervising Pharmacist.

About the Remote Pharmacy Technician

We are seeking a full-time Remote Pharmacy Technician to support our clinical pharmacy program. Working under the direct supervision of a licensed Pharmacist, you will conduct telephonic patient outreach, gather medication histories, and support medication adherence initiatives that drive our quality and cost-of-care measures. This is a fully remote role on a Monday-Friday schedule, with no dispensing or on-site duties.

As part of a growing value-based care organization, this is an opportunity to apply your pharmacy technician skills in a population health setting and work directly alongside clinical pharmacists on medication-related outreach, medication reconciliation, and quality gap closure.

*Must have active Florida licensure.

What You'll Do

• Conduct outbound telephone outreach to patients to obtain, collect, and verify complete medication histories, including prescription medications, over-the-counter products, and dietary supplements
• Conduct accurate and comprehensive medication reconciliation and accelerate medication-related clinical questions or discrepancies to the supervising Pharmacist for review
• Promote medication adherence through proactive patient outreach, including refill reminders, gap-in-care follow-up, and coordination of 90-day prescriptions and mail-order pharmacy transitions
• Assist with quality measure gap closure (e.g., statin therapy, medication adherence) and document outcomes in the EHR and care management platform
• Research and identify medication copay assistance, manufacturer patient assistance programs, and other available affordability options for pharmacist review
• Collaborate with internal patient care team, external healthcare providers, and retail pharmacies to coordinate medication-related needs, resolve access barriers, and support continuity of care
• Maintain accurate, timely documentation in the EHR and generate routine reports for the clinical pharmacy team
• Protect patient privacy and maintain HIPAA compliance in a remote work environment
• Perform other duties as assigned by the supervising Pharmacist

Qualifications of the Remote Pharmacy Technician

• Minimum of three (3) years of pharmacy technician experience, including substantial experience conducting medication reconciliation, obtaining medication histories, and supporting medication therapy management services
• Strong understanding of pharmacy terminology, prescription medications, OTC products, and dietary supplements
• Strong attention to detail and ability to maintain accurate medication and patient information
• Comfort spending the majority of the day on the phone with patients, prescriber offices, and payers
• Exceptional customer service skills with an empathetic, compassionate, and professional demeanor in every interaction
• Strong organizational skills and self-motivation, with the ability to manage a daily outreach queue independently
• Proficiency with information technology, including Microsoft Office (Word, Excel, Outlook) and the ability to become quickly proficient with our EHR and care management platforms
• Tenacious problem solver, with demonstrated capacity to embrace complex problems and arrive at effective solutions in a timely manner
• Enjoys working in a team-based environment with active collaboration across a fully remote clinical team
• Reliable, punctual, and dedicated, with the accountability that remote work requires

Preferred Qualifications

• PTCB certification (CPhT)
• Bilingual — Spanish highly preferred
• Managed care or ACO / value-based care experience
• Experience with Star Ratings, or HEDIS quality measures
• Multi-state registration or eligibility for registration in additional states as required by applicable state regulations

Compensation

Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
