Upperline Health, Inc. is hiring a Remote Pharmacy Technician to support our clinical pharmacy program. You will conduct telephonic patient outreach, gather medication histories, and support medication adherence initiatives from a fully remote setting.

This role requires active CA licensure and involves coordination with pharmacists, care teams, and providers. Work schedule is Monday–Friday with no dispensing or on-site duties, focusing on population health initiatives and quality measures in a

Qualifications

• Active California pharmacy technician registration in good standing.
• Minimum of three years of pharmacy technician experience including medication reconciliation.
• Experience obtaining medication histories and supporting medication therapy management.
• Strong attention to detail and data accuracy in patient records.
• Comfortable spending most of the day on the phone with patients and prescribers.
• HIPAA compliance in a remote work environment.

Responsibilities

• Conduct outbound telephone outreach to patients to obtain, collect, and verify complete medication histories.
• Perform medication reconciliation and escalate questions to supervising Pharmacist.
• Promote medication adherence through outreach, refill reminders, and 90-day prescriptions.
• Assist with quality measure gap closure and document outcomes in the EHR.
• Research copay assistance programs and affordability options for pharmacist review.
• Collaborate with care team, providers, and pharmacies to coordinate needs.
• Maintain accurate documentation in the EHR and generate routine reports.
• Protect patient privacy and HIPAA compliance in remote work.
• Perform other duties as assigned by the supervising Pharmacist.
