Qualifications

• PharmD or Bachelor of Pharmacy required.
• Active U.S. pharmacist license (multi-state preferred).
• Ambulatory care/MTM/clinical experience preferred.
• Excellent communication and ability to build rapport virtually.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
