Job summary

Upperline Health, Inc. is seeking a full-time Remote Pharmacy Technician to support our clinical pharmacy program.

Working under the supervision of a licensed Pharmacist, you will conduct telephonic patient outreach, collect medication histories, and support adherence initiatives in a fully remote setting. This position requires an active California pharmacy technician registration and at least three years of experience, with strong attention to detail and excellent customer service.

Qualifications

• Active California pharmacy technician registration required.
• Minimum of three (3) years of pharmacy technician experience.
• Experience with medication reconciliation and collecting medication histories.

Responsibilities

• Conduct outbound telephone outreach to patients to obtain, collect, and verify complete medication histories.
• Perform medication reconciliation and escalate discrepancies to the supervising Pharmacist.
• Promote medication adherence through patient outreach and adherence initiatives.
• Document outcomes in the EHR and care management platform.
• Maintain HIPAA compliance in a remote work environment.
