Job summary

Upperline Health Plus is seeking a full-time Remote Pharmacy Technician to support our clinical pharmacy program. Working under the supervision of a licensed Pharmacist, you will conduct telephonic patient outreach, gather medication histories, and support medication adherence initiatives that drive our quality and cost-of-care measures.

This is a fully remote role on a Monday-Friday schedule, with no dispensing or on-site duties.

Qualifications

• Current, active California pharmacy technician registration in good standing.
• Minimum of three (3) years of pharmacy technician experience with medication reconciliation and medication histories.
• Strong understanding of pharmacy terminology, prescription meds, OTC products, and dietary supplements.
• Comfort spending most of the day on the phone with patients and payers.
• Proficiency with EHR and care management platforms.

Responsibilities

• Conduct outbound telephone outreach to patients to obtain, collect, and verify complete medication histories.
• Perform medication reconciliation and escalate questions to the supervising Pharmacist.
• Promote medication adherence through refill reminders and 90-day prescription coordination.
• Assist with quality measure gap closure and document outcomes in the EHR.
• Identify copay assistance and patient programs for pharmacist review.
• Collaborate with care teams and pharmacies to resolve access barriers and ensure continuity of care.
• Maintain accurate documentation in the EHR and generate routine reports.
• Protect patient privacy and maintain HIPAA compliance in a remote setting.
• Perform other duties as assigned by the supervising Pharmacist.
