Zusammenfassung

Evergreen Nephrology is seeking a Telephonic Pharmacy Care Coordinator for a 6-month remote contract with potential to convert to full-time. You will engage patients by phone, explain the service, obtain consent, and enroll them in a mail-order pharmacy program with multi-dose pill packs and automatic refill coordination.

You will work with patients from partner practices, update the EHR, and ensure timely access to medications.

Qualifikationen

• Experience with telephonic patient engagement and enrollment.
• Ability to document consent and enrollment in EHR.
• Excellent communication and trust-building over the phone.
• Knowledge of HIPAA and patient privacy.

Aufgaben

• Conduct high-volume outbound outreach to enroll patients.
• Deliver clear, standardized messaging to drive enrollment decisions.
• Obtain and document patient consent by phone.
• Make changes in the EHR based on patient decisions and trigger electronic prescriptions.
• Manage outreach queues and follow-up cadences, completing multiple contact attempts per patient.
• Accurately document all call activity and outcomes in clinical systems.
• Meet call center performance expectations, including call quality and enrollment targets.
• Uphold patient privacy and confidentiality standards (HIPAA).
• Other duties consistent with the role as assigned.
