Job summary

Evergreen Nephrology is seeking a Telephonic Pharmacy Care Coordinator to support a fully remote, patient-facing pharmacy program. This is a 6-month contract with the potential to convert to full-time employment.

You will engage patients by phone, explain the service, capture consent, and guide enrollment as part of a high-volume outreach team. Responsibilities include updating the EHR, coordinating with a mail-order pharmacy, and ensuring adherence to HIPAA while meeting enrollment targets.

Qualifications

• Current Medical Assistant certification (CMA or RMA) required.
• Experience in telephonic or high-volume outbound outreach.
• Strong interpersonal and communication skills for patient engagement.
• Experience in healthcare, pharmacy, or care coordination.
• Proficiency with EHR/EMR and documenting encounters.
• Excellent organization with ability to manage multiple patients and tasks.
• Knowledge of HIPAA and protecting patient privacy.
• MS Office, including Outlook and Teams.
• Ability to work remotely from a dedicated space.

Responsibilities

• Conduct high-volume outbound outreach to enroll patients.
• Deliver clear, standardized messaging over the phone to drive enrollment.
• Obtain and document patient consent by phone.
• Update EHR with patient decisions and trigger electronic prescriptions.
• Manage outreach queues and follow-up cadences.
• Document all call activity and enrollment coordination in systems.
• Meet call center performance targets and KPIs.
• Uphold HIPAA and patient privacy standards.
• Perform other duties as assigned.
• Support remote work requirements and equipment use.
