Job summary

Evergreen Nephrology seeks a Pharmacy Care Coordinator, MA (Contract to Perm) to drive the inaugural pharmacy program. You will engage patients onsite at practice sites in Brooklyn Center, MN, explain the service, capture consent, and document enrollment in the EHR.

The role requires CMA/RMA certification, strong communication skills, and proficiency with EHR systems. It combines in-person care with telephonic enrollment and emphasizes HIPAA-compliant patient privacy.

Qualifications

• Current Medical Assistant certification (CMA/RMA).
• Patient-facing healthcare experience in a clinical, pharmacy, care coordination, or patient support setting.
• Comfort and proficiency working within an electronic health record (EHR/EMR).
• Strong interpersonal and communication skills, with the ability to build trust and engage patients both in person and over the phone.
• Ability to work onsite at practice locations, with reliable transportation as required.
• Working knowledge of HIPAA and a demonstrated commitment to protecting patient privacy and PHI.
• Intermediate skills with MS Office Suite of products including Outlook and Teams.
• Able to work effectively in a primarily remote environment: home internet specs and remote equipment provided by employer.

Responsibilities

• Serve as the onsite Evergreen representative at practice sites, explaining the pharmacy service and providing patient collateral.
• Engage patients with approved messaging to drive enrollment decisions.
• Obtain and document patient consent and enrollment information.
• Update EHR with patient decisions and trigger eRx to the pharmacy partner.
• Conduct telephonic enrollment support for patients without office visits.
• Document all interactions and enrollment activities in clinical systems.
