Qualifications

• High School Diploma or GED required.
• Utah Pharmacy Technician license current and unrestricted.
• National Pharmacy Technician Certification preferred.
• Minimum 1 year of retail pharmacy experience.
• Proficiency in Microsoft Office (Outlook/Excel).

Responsibilities

• Obtain medical records pertaining to recent discharge.
• Navigate multiple EMRs and review labs, H&P, and discharge summaries.
• Outreach to members post-discharge to obtain updated medication lists.
• Coordinate with providers to confirm patient/provider relationships.
• Prepare organized reports for pharmacist review and follow-up with care teams.
