Evergreen Nephrology, LLC seeks a Remote Care Coordinator to enroll patients, coordinate care and address Social Determinants of Health with an interdisciplinary team. You will engage patients by phone, develop care plans, and guide them through the healthcare system.

This role is fully remote and supports patients in the Pacific Time Zone, with standard hours 8:30 am to 5:00 pm PST. Compensation ranges from $19 to $23 per hour, plus eligibility for a bonus and comprehensive benefits.

Qualificações

• 3 years of experience in health care coordination, community health work, or similar healthcare related field.
• Experience with team-based approach to care.
• Excellent working knowledge of community resources.
• Exceptional ability to prioritize and task-switch to meet needs of patients and overall care team.
• Self-directed and motivated to resolve issues professionally and creatively.
• Strong communication and teaching skills.
• Intermediate skills with MS Office Suite of products including Outlook and Teams.
• Ability to work effectively in a primarily remote environment.

Responsabilidades

• Engage patients by telephonic outreach to enroll in the clinical program and serve as the primary contact for enrolled patients.
• Develop and implement care plans addressing patients' needs under the direction of the Nurse Care Manager.
• Facilitate seamless transitions of care across settings to ensure social support and comprehensive care plans.
• Identify social determinants of health and connect members to housing, transportation, food security, and community resources.
• Guide members in understanding and using health and social services to improve access to care.
• Advocate for members by addressing barriers to care and inequities in health access.
• Document interactions, assessments, and care plans accurately in the system.
