Evergreen Nephrology, LLC. is seeking a Telephonic Pharmacy Care Coordinator for a fully remote, 6-month contract with potential to convert to full-time.

The role focuses on patient outreach, enrollment, and EHR documentation for a mail-order pharmacy program serving chronic kidney disease patients. Responsibilities include high-volume outbound calls, script-based enrollment, consent capture, eRx triggering, and HIPAA-compliant data handling.

Qualifications

• Current Medical Assistant certification (CMA/RMA).
• Experience in a telephonic, call-center, or high-volume outbound patient outreach environment.
• Strong interpersonal and communication skills, with the ability to build trust and engage patients over the phone.
• Experience in healthcare, pharmacy, care coordination, or other patient support roles.
• Proficiency with electronic health records; documenting encounters and updating data.
• Strong organizational skills and ability to manage multiple patients and tasks.
• Knowledge of HIPAA and protecting PHI.
• Intermediate MS Office skills (Outlook/Teams).
• Ability to work remotely with a dedicated home workspace and reliable internet.

Responsabilités

• Conduct high-volume outbound telephone outreach to enroll patients in Evergreen's pharmacy service.
• Deliver clear, script-driven messaging to drive enrollment decisions.
• Obtain and document patient consent by phone for enrollment.
• Update EHR with patient decisions, eRx triggers, and consent status.
• Manage outreach queues and follow-up cadences with multi-touch contacts.
• Document all call activity and enrollment steps in clinical/operational systems.
• Meet call center performance targets (quality, productivity, speed, enrollment).
• Uphold patient privacy and HIPAA standards.
• Perform other duties as assigned.
