Job summary

Evergreen Nephrology is seeking a Remote Pharmacy Care Coordinator (MA) to support patients by phone, enroll them in our mail-order pharmacy program, and manage prescription workflows. This 6-month contract can convert to full-time.

Working entirely from home, you will conduct high-volume outbound outreach, document in the EHR, capture consent, and coordinate with the pharmacy partner to ensure timely refills while protecting patient privacy under HIPAA.

Qualifications

• CMA/RMA certification and active credentialing.
• Experience in telephonic, call-center, or high-volume outreach environments.
• Strong interpersonal and communication skills for patient engagement.
• Experience in healthcare, pharmacy, care coordination, or patient support roles.
• Proficient in documenting encounters in an EHR/EMR.
• Solid knowledge of HIPAA and protecting patient privacy.
• MS Office, including Outlook and Teams, in daily use.

Responsibilities

• Conduct high-volume outbound outreach to enroll patients.
• Deliver standardized messaging to drive enrollment decisions.
• Obtain and document patient consent by phone for enrollment.
• Update EHR with patient decisions and trigger electronic prescriptions.
• Manage outreach queues and follow-up cadences with accuracy.
• Document all activity in clinical/operational systems for tracking.
• Meet call-center performance targets for quality and enrollment.
• Uphold HIPAA/privacy standards in all communications.
• Perform other duties as assigned.
