Job summary

Evergreen Nephrology is seeking a remote CCM Care Coordinator to support proactive care coordination for patients with complex chronic conditions, including CKD, hypertension, diabetes, and related comorbidities. You will work under clinical supervision, perform outreach, maintain care plans, and ensure HIPAA-compliant documentation.

This is a fully remote role with Pacific time hours, requiring strong organization, patient communication, and the ability to manage a high-volume panel while

Qualifications

• Certified Medical Assistant credential required.
• Bilingual Spanish/English.
• 1–3 years healthcare experience.
• Chronic disease or nephrology experience preferred.
• Ability to work in a remote environment with HIPAA compliance.

Responsibilities

• Enrollment & eligibility support for CCM patients.
• Conduct outreach via text and phone; support engagement targets.
• Document interactions in CCM platform and EHR; ensure compliance.
• Coordinate referrals and follow-ups; assist care transitions.
• Support care plans and communication between patients and providers.
