Job summary

Evergreen Nephrology is seeking a Remote Telephonic Pharmacy Care Coordinator to support enrollment and coordination for patients with chronic kidney disease. This role operates fully remotely, primarily via phone, coordinating with a specialized mail-order pharmacy and our care team.

You will capture consent, explain the service, and enroll patients while updating the EHR and coordinating with pharmacy partners.

Qualifications

Experience in a telephonic, call-center, or high-volume outbound patient outreach environment.
Strong interpersonal and communication skills are essential.
Experience in healthcare, pharmacy, care coordination, or related patient support roles.
Proficiency using electronic health records (EHR/EMR).
Knowledge of HIPAA and protecting patient privacy is required.
Proficiency with MS Office (Outlook/Teams) and related tools.

Responsibilities

Conduct high-volume outbound telephone outreach to introduce Evergreen's pharmacy service and guide enrollment.
Deliver clear, standardized messaging using scripts and talking points.
Obtain and document patient consent by phone for enrollment.
Update EHR with patient preferences and electronic prescriptions as needed.
Manage outreach queues and follow-up cadences to meet enrollment targets.
Document all activity to support enrollment tracking and reporting.
Meet call center performance metrics including quality, productivity, and average speed of answer.
Uphold patient privacy and HIPAA standards.
Perform other duties as assigned.
