Job summary

Evergreen Nephrology is seeking a remote Chronic Care Management (CCM) Care Coordinator to support care plans for CKD, hypertension, diabetes and related conditions. You will conduct outreach, maintain documentation, and coordinate communication between patients, providers, and partners.

This outreach-driven role emphasizes organization, patient engagement, and HIPAA compliance, with expected high-volume text and calls.

Qualifications

• Certified Medical Assistant credential required.
• 1–3 years of healthcare experience.
• Chronic kidney disease or nephrology experience preferred.
• Strong clinical assessment, communication, and documentation skills.
• Proficient with EHR and CCM platforms.
• Strong organizational and metric-driven workflow management.
• MS Office skills including Outlook and Teams.
• Ability to work in a primarily remote environment with approved home setup.

Responsibilities

• Support CCM enrollment and onboarding activities.
• Verify eligibility based on program guidelines.
• Maintain compliant documentation and HIPAA PHI security.
• Conduct regular patient outreach via text and calls.
• Coordinate referrals and follow-up appointments.
• Document interactions in CCM and EHR systems.
• Ensure engagement time meets Medicare billing requirements.
• Collaborate with remote clinical and operational teams.
• Escalate issues to licensed staff when needed.
