Job summary

Lumicera is hiring a Pharmacist, Patient Consultation Services to provide telephonic clinical consultations for specialty medications. You will assess patients, reconcile medications, educate on disease states and adherence, and collaborate with healthcare professionals to ensure safe, effective therapy.

This part-time role supports two primary coverage models with flexible scheduling. Remote work options are available, with strict adherence to regulatory requirements and Lumicera policies.

Qualifications

• Counsel patients and caregivers regarding medication therapy and disease state management.
• Document patient assessments, interventions, and recommendations in pharmacy systems.
• Evaluate medication regimens to identify drug interactions and adherence barriers.
• Support compliance program objectives and quality improvement.
• Interact cooperatively with patients, caregivers, and health professionals.

Responsibilities

• Conduct telephonic clinical consultations to support safe use of specialty medications.
• Perform patient assessments and medication reconciliation.
• Provide education on medication use, storage, and side effects.
• Answer clinical questions to support informed treatment decisions.
• Escalate clinically significant findings to maintain continuity of care.
• Document consultations in pharmacy systems per policy.
• Maintain productivity and service level standards in the department.
• Identify and report medication errors per procedures.
• Stay current on specialty therapies and guidelines.
• Adhere to laws, regulations, and accreditation standards.
• Perform other duties as assigned.
