Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a bachelor’s degree in Pharmacy or PharmD.
• Current, unrestricted Pharmacist licensure in the resident state or ability to reciprocity within 4 months.
• Experience counseling patients and caregivers regarding medication therapy and disease state management.
• Experience documenting patient assessments and clinical interventions in pharmacy systems.
• Experience evaluating medication regimens for drug interactions and adherence barriers.
• Participate in compliance program objectives and work cooperatively with colleagues.

Responsabilités

• Conducts telephonic clinical consultations to support safe use of specialty medications.
• Performs patient assessments and medication reconciliation to identify barriers and interventions.
• Provides education on disease states, administration, storage, and side effects.
• Answers clinical questions to aid informed treatment decisions.
• Escalates findings through established processes to ensure patient safety.
• Documents consultations and recommendations in pharmacy systems per policies and regulatory requirements.
• Maintains productivity and service level standards through timely patient consultations.
• Supports quality improvement by identifying and reporting medication errors and safety concerns.
• Keeps knowledge of specialty medications and treatment guidelines up to date.
