Job summary

Lumicera is seeking a Pharmacist, Patient Consultation Services for remote, part-time coverage. The role provides telephonic clinical consultations to specialty-pharmacy patients on complex therapies.

You will perform patient assessments, reconcile medications, educate patients and caregivers, and document findings in our systems. The position requires ACPE-accredited pharmacy education, current pharmacist licensure, and experience in patient counseling and medication management.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a bachelor’s degree in Pharmacy or PharmD required.
• Current, unrestricted licensure as a Registered Pharmacist in the state of residence, or reciprocity within 4 months may be required.
• Experience counseling patients and caregivers regarding medication therapy and disease state management.
• Experience documenting patient assessments and interventions in pharmacy systems.

Responsibilities

• Conducts telephonic clinical consultations to support safe and effective use of specialty medications.
• Performs patient assessments and medication reconciliation to identify adherence barriers and drug interactions.
• Provides education on disease state management, administration, storage, and side effects.
• Answers clinical questions from patients and caregivers to support informed treatment decisions.
• Documents consultations and interventions in pharmacy systems per policy and regulatory requirements.
• Maintains productivity and quality by timely completion of patient consultations.
• Supports quality improvement by reporting medication safety concerns.
