Job summary

Trillium Health Resources is seeking a Peer Support Specialist - In Reach to provide peer-based support for individuals navigating care transitions and engagement in services. The role connects clients to resources, promotes recovery, independence, and community integration.

Responsibilities include outreach, navigation of services, facilitating groups, and encouraging active participation in care, with a focus on lived experience to build trust.

Qualifications

High school diploma or GED required; one year lived experience in behavioral health or recovery.
Valid driver’s license required.
Residence within Trillium’s Mid State Region counties.
Ability to travel within catchment as needed.
Authorized to work in the United States without sponsorship.

Responsibilities

Provide peer-based support using lived experience to engage individuals in services.
Support outreach to connect individuals with community resources.
Assist individuals in navigating services and accessing supports.
Facilitate group activities, educational sessions, and community engagement.
Promote self-advocacy and independence in service participation.
