Job summary

Trillium Health Resources is seeking a Peer Support Specialist - In Reach to provide peer-based support for individuals navigating care transitions and engagement in services. The role connects clients to resources, promotes recovery, independence, and community integration.

Qualifications

• High school diploma or GED required; one year lived experience in behavioral health or recovery.
• Valid driver’s license required.
• Residence within Trillium’s Mid State Region counties.
• Ability to travel within catchment as needed.
• Authorized to work in the United States without sponsorship.

Trillium Health Resources has a career opening for a Peer Support Specialist to join our team! The Peer Support Specialist - In Reach provides peer-based support to individuals navigating care transitions, using lived experience to build trust and promote engagement in services. The role connects individuals to resources, supports participation in care, and promotes recovery, independence, and successful community integration.

On a typical day, you might:

• Provide peer-based support using lived experience to build trust and support engagement in services.
• Support outreach and engagement efforts to connect individuals to services and community resources.
• Assist individuals in navigating services and accessing available supports.
• Facilitate group activities, educational sessions, and community engagement opportunities.
• Promote self-advocacy, independence, and active participation in services.

Qualifications

Required:

• High school diploma or GED and one (1) year lived experience with a behavioral health condition and/or substance use. *Lived Experience is defined as living in recovery with mental illness and/or substance use disorder.
• Must have a valid driver’s license.
• Must reside within Trillium’s Mid State Region, which includes the following counties: Anson, Guilford, Montgomery, Randolph, and Richmond.
• Must be able to travel within catchment as required.
• Must be legally authorized to work in the United States without requiring sponsorship for employment now or in the future (ex: OPT F-1, J-1).
