Job summary

Trillium Health Resources is hiring a Peer Support Specialist to provide peer-based support for individuals navigating care transitions, using lived experience to build trust and encourage engagement in services.

The role connects individuals to resources, supports participation in care, and promotes recovery, independence, and successful community integration in a remote-capable position across the Southern Region.

Qualifications

• High School Diploma or GED is required.
• Minimum 1 year lived experience with a behavioral health condition and/or substance use.
• Must reside within Trillium's Southern Region and be able to travel within the catchment.

Responsibilities

• Provide peer-based support using lived experience to build trust and promote engagement in services.
• Assist individuals in navigating services and accessing available supports.
• Support outreach and engagement efforts to connect individuals to services and community resources.
• Facilitate group activities, educational sessions, and community engagement opportunities.
• Promote self-advocacy, independence, and active participation in services.
