Job summary

Trillium Health Resources is hiring a Peer Support Specialist - In Reach to provide peer-based support to individuals navigating care transitions using lived experience.

The role connects people to resources, supports engagement in services, facilitates group activities, and promotes recovery, independence, and successful community integration. Office location is remote within the Mid State Region, serving Anson, Guilford, Montgomery, Randolph, and Richmond counties.

Qualifications

• High school diploma or GED and one (1) year lived experience with a behavioral health condition and/or substance use.
• Must have a valid driver's license.
• Must reside within Trillium's Mid State Region, which includes Anson, Guilford, Montgomery, Randolph, and Richmond counties.
• Must be able to travel within catchment as required.
• Must be legally authorized to work in the United States without requiring sponsorship for employment now or in the future.

Responsibilities

• Provide peer-based support using lived experience to build trust and support engagement in services.
• Support outreach and engagement efforts to connect individuals to services and community resources.
• Assist individuals in navigating services and accessing available supports.
• Facilitate group activities, educational sessions, and community engagement opportunities.
• Promote self-advocacy, independence, and active participation in services.
