Trillium Health Resources is seeking a Peer Support Specialist to join the Support Transition and Reentry (STAR) team. The role provides transition care coordination for adults exiting incarceration, guiding navigation of behavioral health services, housing, employment, and other supports.

You will work with a multidisciplinary team to connect recipients with pre-release and post-release services, ensuring warm handoffs and ongoing support in the community.

Qualifications

• Required: High School Diploma or GED.
• One year lived experience with a behavioral health condition and/or substance use; lived experience defined as living in recovery.
• Must have a valid driver’s license.
• Must reside in North Carolina within 75 miles of Trillium’s Southern Region, specifically New Hanover County.
• Must be able to travel within catchment as required.
• Must be legally authorized to work in the United States without requiring sponsorship for employment now or in the future.”

Responsabilités

• Meet with Justice Involved members in-person in Correctional Facilities and in the community to facilitate care management activities.
• Provide transition linkages including housing, employment, and community resources.
• Educate members about navigating the criminal justice, behavioral and social services systems.
• Assist with transition planning and ensure delivery with focus on quality outcomes.
• Engage with area justice systems including jails, prisons, probation and parole and pre-release officers.
