Job summary

Oak Street Health is seeking a Remote CC Service Advocate I to manage appointment scheduling and patient interactions, delivering courteous service and accurate data entry in a remote setting.

You will handle inbound and outbound calls, coordinate with medical staff, follow up to confirm appointments, and maintain documentation while supporting 40-hour weekly schedules and 24/7 center operations.

Qualifications

• High School diploma or equivalent required; some undergraduate education preferred.
• 1 year of customer service experience; call center experience is a plus.
• Flexible and positive attitude toward patients and team.
• Friendly, nurturing approach to older adults.
• Experience with EMR documentation preferred.
• Ability to multitask, prioritize, and manage time effectively.
• Outstanding phone demeanor and etiquette.
• High level of integrity and reliability.
• Proficient PC skills including basic Excel usage.
• Spanish, Mandarin, Cantonese, or Polish speakers preferred.
• U.S. work authorization required.

Responsibilities

• Handle inbound and outbound calls to schedule, reschedule, and cancel patient appointments.
• Provide courteous and professional service to patients and address inquiries.
• Accurately enter patient information and appointment details into the system.
• Clearly communicate appointment details and any necessary instructions to patients.
• Identify and resolve scheduling conflicts promptly.
• Collaborate with medical staff and departments to ensure smooth operations.
• Conduct follow-up calls to confirm appointments and send reminders.
• Maintain accurate records of all interactions and transactions.
