UnitedHealth Group is seeking a Patient Outreach & Scheduling Coordinator to support the Complex Care team by coordinating appointments and proactive outreach. You will manage outreach lists, schedule and reschedule visits, and help ensure patients connect with the right clinical resources.

This remote role requires strong organization, experience with Epic or similar EHRs, and the ability to handle high volumes while protecting patient privacy.

Formación

• High school diploma or GED required.
• Experience with an electronic health record, preferably Epic.
• Ability to manage a high volume of outbound patient contacts with accuracy.
• Strong organizational skills to manage multiple worklists, providers, clinics and priorities.
• Comfort learning new technology across multiple systems.

Responsabilidades

• Conduct outbound outreach to patients for clinical services and appointments.
• Schedule patients with the appropriate provider, clinic location, visit type and time.
• Encourage telehealth where applicable and perform reminder outreach.
• Assist with rescheduling patients affected by provider changes or clinic cancellations.
• Review provider schedules to ensure proper patient placement and coordinate with leadership.
• Document outreach attempts and scheduling outcomes in Epic and Smartsheet.
