Qualifications

• Associate’s degree or two years of healthcare coordinator experience.
• Minimum of 3 years of experience in healthcare coordination or an administrative role.
• Experience with Epic scheduling, referrals, and MyChart communications.

Responsibilities

• Coordinate patient care across providers, specialties, and settings for chronic/high-acuity cases.
• Serve as liaison between patients, internal and external physicians and clinical teams; triage calls.
• Manage surgery scheduling and case requests; ensure proper documentation.
• Oversee referrals and prior authorizations for tests, imaging, procedures.
• Coordinate with external providers for records, images, and labs.
• Maintain clinic reports, waitlists, and patient-facing documentation.
• Educate patients on procedures, resources, and financial requirements via MyChart.
• Manage Epic work queues for scheduling and outreach workflows.
• Scrub clinic schedules for procedural accuracy and record receipt.
• Support patient needs with internal/external programs.
