Health Advocate is seeking a Patient Navigator for a fully remote, evening-shift role in the United States. The role focuses on outbound patient outreach, guiding patients through care steps, scheduling, and resolving barriers to access.

The ideal candidate has a high school diploma, 2+ years in patient support, and excellent communication and confidentiality skills. Compensation is $18.00/hour base plus evening differential.

Qualifications

• High School Diploma or GED is required.
• Associate’s degree in healthcare, social work, business administration, liberal arts, or related field is preferred.
• 2+ years of experience in customer service, healthcare navigation, or patient support is required.
• Experience in care coordination, community resources, or healthcare access is preferred.
• Strong communication skills and active listening abilities are essential.
• Comfort with documenting across multiple systems and maintaining confidentiality.

Responsabilités

• Handle high-volume outbound patient outreach with professionalism and empathy.
• Explain next steps and processes in clear language.
• Assist patients with appointment scheduling and care coordination.
• Identify barriers to care and connect patients with resources.
• Document conversations accurately and securely.
• Escalate when clinical expertise is needed and coordinate with clinical resources.
