Qualifications

• Proficiency in insurance eligibility verification and benefit navigation.
• Experience in healthcare, patient services, or social services preferred.
• Travel up to 25% of time as needed.

Responsibilities

• Assist patients with Medicaid/Medicare/ACA navigation and enrollment.
• Coordinate transportation and connect with community resources.
• Support patient engagement, retention, and re-engagement efforts.
• Document interactions and outcomes per policies and procedures.
