Health Advocate is seeking a Patient Navigator to conduct high-volume outbound outreach and guide patients through healthcare next steps. You will discuss sensitive information with empathy, document accurately, and coordinate care with HIPAA-compliant practices.

The role emphasizes evening shifts (1:00 PM–9:30 PM ET or 1:30 PM–10:00 PM ET), a base rate of $18.00/hour plus applicable evening differential, and a remote work setup within the United States.

Qualifications

• High School Diploma or GED required.
• 2+ years in customer service, healthcare navigation, or patient support.
• Associate’s degree preferred or related field advantageous.
• Experience in care coordination or healthcare access preferred.
• Strong communication and active listening abilities.
• Able to document across multiple systems accurately.
• Commitment to confidentiality and compliance (HIPAA).

Responsabilités

• Conduct high-volume outbound patient outreach.
• Explain next steps and processes in clear language.
• Help patients navigate appointment scheduling and care coordination.
• Identify barriers and coordinate resources until next steps are set.
• Document conversations accurately and timely.
