Optum is seeking a patient coordination professional to welcome and orient prospective patients, coordinate delivery of health enhancement services, and act as the primary contact for the Health Enhancement/Education team across the care continuum.

The role supports remote work from anywhere in the U.S. with in-office requirements in Minneapolis or Washington, D.C. for some hires. Strong communication and multitasking skills are essential.

Formación

• High school diploma, GED, or equivalent.
• Proven exceptional English written and verbal communication skills.
• Healthcare experience or exposure preferred; ability to coordinate patient-centric care.

Responsabilidades

• Welcome and orient prospective patients to health enhancement programs and coordinate service delivery.
• Serve as primary contact to support Health Enhancement/Education team and ensure continuity of care.
• Contact patients to participate in programs and document correspondences.
