Job summary

Savista is hiring an Eligibility Specialist 2 to support patients remotely with complex eligibility screenings for Medicaid, Medicare, disability, and charity programs. You will conduct in-home visits when necessary, guide families through applications, and coordinate with hospitals and agencies to secure coverage.

Qualifications

• Proficiency in English and Spanish.

Responsibilities

• Serve as a patient advocate, conducting advanced screenings to assess eligibility for financial programs.
• Perform in-home patient visits to support completion of applications.
• Engage patients via phone outreach to facilitate screenings and documentation.
• Guide patients through Medicaid, Medicare, Disability, hospital charity care applications.
• Independently manage application follow-up across multiple programs.
• Prioritize tasks, meet deadlines, and maintain productivity and quality KPIs.
• Support disability applications and appeals, coordinating appointments and submissions.
• Collaborate with hospital staff and government agencies to secure coverage.
• Assist onboarding and training of colleagues, sharing knowledge.
• Explain financial responsibilities clearly and empathetically to patients.
• Maintain confidential records and HIPAA/compliance adherence.
• Use multiple systems to document, track, and report patient information.
• Provide coverage across nearby locations as needed and support in-home visits.
• Complete special projects as assigned.
