Job summary

TridentCare is seeking a Remote Patient Success Coordinator to support home-based orders and ensure on-time service delivery while delivering excellent customer experiences for patients and providers. This role requires strong communication and coordination across teams, with a focus on quality and timely follow-up.

The position is part-time, weekend-oriented (Saturday and Sunday, 8:00am–6:30pm EST) with a pay range of $17.00–$19.00 per hour.

Qualifications

• High School degree or GED required.
• Associate's degree preferred.
• Medical terminology experience preferred but not required.
• Ability to type 35-40 words per minute.

Responsibilities

• Work directly with the field leadership team to ensure timely services fall within the service matrix.
• Assist with scheduling of services and communicate with patient and ordering entity.
• Provide ETAs to patient or ordering entity where applicable.
• Reschedule and cancel exams when appropriate and communicate with patient or ordering entity.
• Prioritize dispatching of technicians based on client SLAs with internal field operations.
• Become SMEs in various TridentCare at Home client models including Hospital at Home, ED at Home, and PACE programs.
• Coordinate with the internal teleradiology team to ensure successful image transfer processes.
• Address patient/client questions and concerns promptly; record for Quality Improvement metrics.
• Assist in obtaining required paperwork for timely billing as applicable.
• Monitor reporting and operational metrics for all home-based services.
• Answer incoming customer service orders as soon as possible.
• Demonstrate active listening and superior customer service for external and internal customers.
• Enter orders manually via our fax process where needed.
• Navigate TridentCare applications (TCC, DDF, etc.).
• Operate the phone system effectively.
• Adhere to work schedule.
